Report of Hazard or Unsafe Condition
The Report of Hazard or Unsafe Condition is designed to encourage employee participation in the Safety and Health of the Panama-Buena Vista Union School District to ensure prompt action when employees report a hazard.
This form should not be used to report immediate and dangerous working conditions. If a dangerous working condition exists that requires immediate corrective action, the employee shall notify his/her supervisor at once.
Please note this report may be submitted anonymously.

Employees may submit this form through a Google Form or can access the form below. The form can be submitted at the the Health, Safety and Wellness office. District mail may also be used for anonymous notification.
